【Abstract】 Objective To study the effect of Glucose-Insulin-Potassium(GIK)and metoprolol on the auˉtonomic nervous system and sudden cardiac death of patients with acute myocardial infarction(AMI).Methods 129patients with AMIwereat random divided into four groups:GIK group(n=37),metoprolol group(n=32),concomitant GIK and metoprolol group(n=30)and control group(n=30).30patientswere given routine treatment as a control group,the patiens of treamtnet groups were given routine treamtnet plus GIK,metoprolol,concomitant GIK and metoˉprolol.The patients were undergone24hour ambulatory electrocardiogram monitoring and investigated the changes of HRV,ventricular arrhythmia,mortality,heart function before and after treatment.Results The HRVwere of no signifˉicant changesbefore treatment between these groups.The HRV were significantly improved.Ventricular arrhythmia(lown≥Ⅲ)were decreased after7days of medical treatment withGIK group21.6%(8/37),metoprolol group18.8%(6/32),concomitant GIK and metoprolol group10.0%(3/30),compared with the control group63.5%(19/30),which had significant difference(P<0.01).A statistically significant reduction in heart failure and mortality after treatment in concomitant GIK and metoprolol group(P<0.01).Conclusion Both GIK and metoprolol can improve the HRV,heart function and significantly reduce the malignant ventricular arrhythmia,heart sudden death after AMI.There is better effect when the two drugs were used in combination.

1.2 方法 对AMI入选患者发病后24h内进行常规Holter检查,分析HRV,测定心功能(EF)。治疗第7天、1个月复查HRV和EF。
