食管胃后壁两定点前壁Gambee氏吻合法的临床应用

　　【摘要】　目的：探讨降低食管癌、贲门癌切除术后吻合口瘘及狭窄发生率的手术方法。方法：对120例病人采用食管胃后壁两定点前壁Gambee氏吻合法，并与同期采用传统双层包套式吻合法的88例相比较。结果：本组无吻合口瘘及狭窄发生，而传统吻合法颈部瘘发生率7.4%(4/54)，胸内瘘发生率为5.9%(2/34)。吻合口狭窄10例。结论：食管胃后壁两定点前壁Gambee氏吻合法是一种操作简便，易于掌握，能有效防止吻合口瘘及狭窄的吻合方法，值得推广应用。

The Clinical Application of A Method of Esophago gastric Anastomosis-the Posterior Wall with Two Selected Sites and the Anterior with Gambee's Suture

　　【Abstract】　Objective：To discuss the relationship between the methods of esophago gastric anastomosis and the occurrence of the postoperative anastomotic leakage and stenosis in the patients with esophageal and cardiac cancer.Methods：The recommended method was used in 120 cases while the traditional method-double layers purse-string suture in another group (88cases). The incidence of the postoperative anastomotic leakage and stenosis was compared in two groups.Results：The Gambee′s grounp has no complications of anastomotic leakage and stenosis compared with the rate of anastomtic leakage and stenosis 5.9% in the traditional group.Conclusions：The recommended method of esophago gastric anastonosis could effectively prevent the complications of postoperative anastomotic leakage and stenosis.
