圆酵母样亨德逊霉引起皮肤真菌病国内首例报告

　　【摘要】报告国内首见圆酵母样亨德逊霉所致头癣、体癣1例。患者女性，68岁，干部，曾旅居过缅甸、泰国。主诉头皮出现脱屑性斑片并断发、痒3个月，皮损向左侧延及耳轮、耳背，右肩出现脱屑性斑片，痒3周。以上皮损处取材镜检均见粗细不等之淡棕色分支分隔菌丝，关节菌丝。断发镜检见发外孢子、发内关节菌丝。皮损处多次取材培养，均为同一菌落。葡萄糖蛋白胨琼脂基、27℃培养，菌落生长良好。接种3d后,葡萄糖蛋白胨琼脂基可见白色绒毛状菌落，10d后逐渐变为灰褐色绒毛状菌落，背面为黑褐色，镜下见粗细不等之分支分隔菌丝、关节孢子、棕色厚壁孢子及烧瓶状分生孢子梗。关节孢子特征呈双轮廓。在含有放线菌酮葡萄糖蛋白胨琼脂基上菌落未见生长。鉴定为圆酵母样亨德逊霉(Hendersonulatoruloidea)。本文首次用实验证明该菌可侵犯毛发。在血琼脂基37℃培养，菌落呈酵母样，镜下见孢子形成的假菌丝。首次提出该菌为双相型真菌。

Dermatomycosis Caused by Hendersonula toruloidea:First Case Report in China

　　【Abstract】 Objective To report a rare case of tinea capitis and tinea corporis caused by Hendersonula toruloidea. Methods The specimens (scales) were taken from the scalp, ear and right shoulder of a 63－ year－ old female patient. The fungus was identified according to direct microscopy, and the characteristic morphology of the colony in the culture. Results The pathogenic fungus in the case was identified as Hendersonula toruloidea. Conclusion Hendersonula toruloidea could be a pathogenic fungus of tinea capitis and tinea corporis, and it is dimorphic fungus.

　　直接镜检：取头皮、耳及右肩皮屑加1.78mol/L(10％)KOH后直接镜检，见粗细不等分支分隔菌丝，有的菌丝为淡棕色，可见关节菌丝。取断发镜检，见发外大量关节孢子，发内关节菌丝密集，并见双轮廓孢子（图3）。
