线性IgA大疱病基底膜带自身抗体的靶抗原异质性

　　【摘要】目的了解线性IgA大疱病基底膜带自身抗体（BMZ－Ab）靶抗原及其与其它表皮下自身免疫性大疱病相关抗原的关系。方法用热分离法提取真表皮抗原，免疫印迹检测11例线性IgA大疱病中IgA型和IgG型BMZ－Ab靶抗原的分布情况。结果11例线性IgA大疱病患者中9例血清中IgA型BMZ－Ab可分别与真、表皮抗原结合，其中7例结合97000表皮抗原（LABD97），2例同时结合180000抗原，另有2例成人型线性IgA大疱病分别结合180000或230000表皮抗原。5例IgG型BMZ－Ab结合表皮97000、145000、180000、230000抗原，未见与真皮抗原发生反应。结论线性IgA大疱病中BMZ－Ab的靶抗原表现为异质性，除LABD97以外，还可以是BP180、BP230等大疱性类天疱疮（BP）相关抗原。LABD97可能是BP180分子的降解产物，线性IgA大疱病是否是一种独立疾病应深入研究。

Heterogeneity of Target Antigens of Basal Membrane Zone Antibodies in Linear IgA Bullous Dermatosis

　　【Abstract】 Objective To detect the target antigens of basal membrane zone antibodies (BMZ－ Ab) in linear IgA bullous dermatosis (LABD). Methods Dermal and epidermal antigens were extracted by heat－ separated procedure, then the target antigens of BMZ－ Abs in 11 LABD sera were detected. Results The IgA BMZ－ Abs in 9/11 LABD sera could reacted with dermal and epidermal antigens respectively. 7/11 LABD sera reacted with 97 000 epidermal antigen (LABD 97), 2 sera reacted with 180 000 epidermal antigen simultaneously. 2 adult LABD sera recognized 180 000 and 230 000 epidermal antigen respectively. IgG BMZ－ Ab in 5/11 LABD sera reacted with 97 000, 145 000, 180 000, 230 000 epidermal antigens, and did not react with dermal antigens. Conclusions The target antigens of IgA BMZ－ Abs in LABD were heterogeneous including LABD 97, BP 180 and BP 230. LABD 97 may be a degraded fragment of BP 180. Whether LABD is an independent entity or not should be further investigated.
